I recently made a pattern using 22ct fabric. Normally 22ct is stitched across 2 threads (i.e. one stitch covers a 3x3 area instead of a 2x2 area). When calculating the size, in inches, for the pattern, does PCStitch automatically assume sewing across 2 threads since I'm using 22ct fabric?

No,using a count of 22 assumes "over one". If you want to do 22 count over 2 then you need to specify 11 count.
