What am I doing wrong?

The pattern itself may be fine. What parameter do you have set beside the "preview" box? Try clicking "To Fit" to see what happens.
Similarly, notice in the second image, the grid is showing only a narrow part on the top.
Note -- for the best import, convert your raw image to the SAME pixel count as the stitch count you want. You give a size of 340 X 610 - then convert your image to the same count IN PIXELS - not inches, centimeters or anything else, just the image you see in pixels. it may look tiny but a 1:1 ratio of pixels to stitches gives the best starting point for an import.
