I created a pattern that use both 2 plys sometimes it's 2 threads of the same colour and sometimes it is a blend of two different colours.

I then printed out the pattern and information sheet. I am having trouble understanding this information sheet.

It list the colour multiple times most of these times it has an "*" beside the number. Are the ones listed with an "*" the one used in the blends?

Yes, that is what the asterisk means. Notice the section that shows the blends. It clearly marks (*) Blended Entries: to show the mapping.
